PRESCHOOL REGISTRATION REQUIREMENTS
Please complete the attached forms below and send one completed packet to Sharon Leight at sleight@obps.org or call (732) 360-4462 to make an appointment to drop off your forms. Everything is required with the exception of a physical. That must be completed within 30 days of starting the program. Please note there are different requirements based on whether you own your home, rent your home, or live with a family/friend.
