
Registration is a two Step Process:
Step 1 – Complete the pre-registration.
Step 2 – Submit registration packet and supporting documents (available at the end of the pre-registration).
Reminders:
Children are registered in their zoned schools in order that completed registrations are received. Please be advised if grade-level capacity is reached, incoming students may be assigned to another Old Bridge Elementary School. In the event of an overflow situation, you will be advised of your child’s attending school.
Incomplete packets received will not be processed and you will be notified.
Affidavits must be notarized.
Vaccinations and physical must be signed by the doctor and submitted with all the other documents.
Copy of the birth certificate will be accepted.
Proof of residency must be included.
If dropping packet in the drop box, please do not submit originals. All copies needed must be made beforehand.
All foreign documents must be translated.

If you child has an IEP, a copy of the valid IEP must be submitted with the registration packet.

Sports Clearance - Grades 6 - 12
All original Athletic Pre-Participation Physical Examination Forms must be handed in at the high school for sports clearance. Failure to hand the originals can delay the approval for your child to try out for a specific sport.
Please include a copy of the Athletic Pre-Participation Physical Examination Form with your registration packet.
Deadline for Sports Approval

Winter Sports - October 15th
Spring Sports - February 1st
